Pursuant to the Paperwork Burden Reduction Act and recent IRS guidance (IRS Notice 2025- 15), Earlville CUSD9 is utilizing an alternative method for furnishing Form 1095-C.

We will not mail paper copies of Form 1095-C to your home address.

What is Form 1095-C? Form 1095-C provides information about the health insurance coverage offered to you by Earlville School District. While this form is related to your taxes, it is not required to file your 2025 federal income tax return. You may simply indicate on your return that you had health coverage.

How to Request a Copy If you would like to receive a copy of your Form 1095-C for your records, you have the right to request one at any time. We will provide the form to you within 30 days of your request.
